Registration payment, in U.S. Dollars only, is due upon submission of registration. The registration fee includes softbound conference proceedings, welcome reception, breaks, banquet, and a 20% non-refundable cancellation fee. All requests for refunds must be received in writing no later than May 24, 2004, and will be assessed a 20% cancellation fee. No refunds will be made after that date. A $50.00 fee will be charged for all substitutions.

Credit Hour Statement:
Northwestern University, Feinberg School of Medicine designates this educational activity for a maximum of 19.5 category 1 credits toward the AMA Physician's Recognition Award. Each physician should claim only those credits that he/she actually spent in the activity.

CME Certificates:
In order to obtain your CME certificate for this activity, it will be necessary for you to complete the Credit Hour Form that is included in your conference materials. Forms should be completed in full and returned to the registration desk at the end of the meeting. Certificates will be mailed 3-4 weeks following the conference to the address provided on your registration form.

Non-US Physicians:
The American Medical Association has determined that physicians not licensed in the US who participate in this CME activity are eligible for AMA PRA category 1 credit.
